Thank you for choosing Adoption Support Services of Florida to complete your home study in the state of Florida. Below you will find a list of items to complete the home study. Please note the directions for each form, or click on the link to download the form from this site.
Client Information Form. Please fill out the form completely and include copies of the documents listed below. Certified copies are not necessary.

Birth Certificate for each applicant. You may use a copy for your home study, but a certified copy may be necessary for the dossier.

Marriage License. (If applicable.) You may use a copy for your home study, but a certified copy may be necessary for the dossier.

Divorce Decree. (If applicable.)

Naturalization Papers. (If applicable.)

Copy of Heath Insurance Card.

Federal Income Tax Return Form from previous year (1040, first 2 pages, Adjusted Gross Income is what we’re looking for).

Floor Plan of House. This can be hand drawn with rooms labeled.
- Color Family Photo. Photo of everyone living in household. A color copy is fine.
Life Summary. Please type your life summaries and answer ALL of the questions completely. Your life summaries are to be submitted with your Client Information form and Financial Statement prior to your home visit.
Financial Form. This financial form is to be submitted with your Client Information form and Life Summaries prior to your home visit.
Department of Children and Families Background Clearance. One per person over the age of 12 then submit to Adoption Support Services.
FDLE VECHS Waiver Agreement and Statement. One per person over the age of 12.
FDLE/FBI Background Checks One per person, 12 years of age or older. Read instruction carefully. You will need to have this clearance done even if you have recently had it done through another source.
Local County Law Enforcement Background Check – ALL household members 12 years of age or older will need to have a local back ground check done for the county you live in. Your background must include all names (maiden, current, and aka’s). You do not need to be fingerprinted for this background check. There is no link for this to click on. You just need to contact your County Sheriff’s office for their procedure.
Five Personal Reference Letters. You will need to obtain five (5 total) personal references. One may be from a family member. The references you choose should know each applicant. Give this form to your references as a guide to help them cover the topics needed in their letter. Please have them create a letter, sign, and then email back directly to me as a PDF with (your name reference) in the subject line. DO NOT have them write to me in the body of an email. It needs to be an attached document.
Physician Form. Complete for everyone in the home 18 and older. Please have a physician fill out this form completely. *If you have children in your home under the age of 18 please send a copy of their most recent physical/immunization record or a letter from their pediatrician indicating the child’s health status.
Employment Verification Letter. This information must be on company letterhead. If you are self-employed, you will need to obtain a letter from your accountant stating what your personal earnings are or income from previous year.
Affidavit of Good Moral Character Each member of the household who is 18 years or older will need to complete the form. Please fill in the county where you reside, your full name and sign either the first statement or the second statement on page 3 in the presence of a notary and have them complete the information on the bottom of page 3.
This form is not specific to adoptive parents, however, the Department of Children & Families requires the use of the form for Foster Care and Adoption Applicants. In the blanks where is says “employee of” fill in Adoption Support Services.
Guardianship Statement. Please have your designated guardians sign this form.
Firearms Safety Form. Please read and sign this form.
Adoption Education. Click on the link and follow the instructions. If you already have an adopted child, this requirement is waived.
